Passed away quietly at home on Monday October 29, 2012 at the age of 50 years. Beloved son of Vladimir and Phyllis nee Erhardt.Dear brother of Roxanne Tim Carloni, Sandi Perich, Steven Sonia Perich, Tom Perich and Natalie Dockerty. Fond uncle of 7 nieces and 3 nephews. Special friend of Sandie Perich and Bob Dockerty. Also survived by other relatives and friends.

Visitation will be held on Friday Nov. 2, 2012 at the FUNERAL HOME from 9:30AM until the time of Service 10:30AM. Interment Arlington Park Cemetery. The family wishes to thank the caring staff of Bell Therapy Day One.
