Passed away Monday, January 11, 2016 age 83 years. Preceded in death by his beloved wife Patricia (nee Koscielak), daughter Sandra, and son James.
Loving father of Julie Cook. Dear brother of Randy Jr. (Doris), the late Don (late Adele), Morrie (Pat) and the late Roger Cook. Further survived by nieces, nephews, other relatives and friends.

Visitation Thursday, January 14 at the funeral home from 4-7 PM with a funeral service at 7 PM.
Interment St. Adalbert’s Cemetery Friday at 11 AM. Jerome was a Retiree of INRYCO.
