Born to eternal life September 22, 2009 at the age of 64 yrs. David Erich partner in life. Dear mother of Diana Andrew Mathis Jacobsen, Keith Linda Jacobsen, and the late Glenn. Loving grandma of Tiffany and Taylor Jacobsen. Beloved sister of Timothy Lally. Also survived by other relatives and friends.

Visitation will be held on Monday, at the funeral home, from 4-7 PM. A Memorial Service will follow at 7 PM.
